1. Job Purpose

1.1 To provide a direct service of a supportive and practical nature to disabled adults within the Assessment and Care co-ordination service

1.2 Undertake assessment of need for non complex cases, identify need, develop and implement an appropriate care plan.

1.3 Undertake reviews of people in residential/nursing care, stable complex care packages and non complex cases in line with the departments procedures

1.4 Facilitate move from residential/nursing home to residential/nursing home or any combination of these in line with the need of the individual.

1.5 Ensure that placements and care packages are able to meet the needs of the
individual and are person centred

1.6 Ensure high quality monitoring and case management of all people allocated.

1.7 With the placements teams support, negotiate appropriate placements ensuring
that the placement can meet the needs of the service users and are cost
effective

1.8 Write report and present cases to the budget holder or panel for funding
agreement.

1.9 To positively participate in the service and departments monitoring systems.

1.10 To undertake either all or some of the aspects within this role in a flexible
manner as directed by the manager to meet service requirements.

1.11 To undertake these duties within agreed departmental service objectives, policies and procedures and to promote the Council's Equal Opportunity Policy and the principles of non-discriminatory practice.

2. Community Case Work

2.1 Acknowledge referrals for community or hospital assessment and undertake gathering of initial information

2.2 Arrange visit for assessment or reassessment and undertake the assessment of
need using the appropriate assessment tool.

2.3 Following assessment visit, develop care plan to meet needs and present the assessment and care plan to the budget holder or panel for funding agreement. Inform service user, home and family, where appropriate, of outcome of assessments in writing.

2.4 Undertake any other tasks identified in the assessment and refer on or signpost
to other services as required.

2.5 Implement care plan and set up services using the appropriate departmental procedures and systems. Ensure service user has a copy of the care plan

2.6 Review cases in line with the department procedures and as directed by the
manager or duty manager.

2.7 Undertake other work such as, but not exclusive to one off tasks such as
emergency payments, charity and grant applications or ongoing case work as
required by the manager or duty manager.


3. Placements

3.1 Review placements in line with the services policies, procedures and
priorities.

3.2 Arrange face to face reviews ensuring that the service users, their family, the key worker, other key professionals and/or any other person invited by the service user can attend.

3.3 Attend, chair and where necessary minute reviews. Ensure that the review is a person centred as possible, enabling the service user space to express themselves.

3.4 Inspect premises including bedroom (if agreed by service user), kitchens, toilets and communal areas. Inspect financial, medication and other records as appropriate.

3.5 Write up the review, outlining outstanding actions and distribute them within the policy timescale.

3.6 Undertake phone or paper reviews as outlined in the service policy and procedures.

3.7 Agree timescale for next review with the manager or duty manager.

3.8 Where agreed with manager or duty manager report any concerns regarding registered care to CSCI.

3.9 Identify actions required with timescales and identified workers to undertake the action.

3.10 Review action on a 6 monthly basis unless an earlier timescale is agreed with the manager or duty manager.

3.11 Consider all possibilities to prevent placement breakdowns. Undertake a face
assessment if required. Request written reports from appropriate agencies
where necessary.

3.12 For a change of placement write an assessment report and present all relevant
assessments to the panel through the agree procedures. Where required attend
panel and undertake any follow up action as required. Inform service user,
home and family, where appropriate, of outcome of assessments in writing.

3.13 Where a new placement is required liaise with the placement team to find
suitable placement. Support the service user to move to new accommodation
or through the changes to the care package as agreed by the manager or duty
manager. Involvement in this will vary depending on individuals personal
circumstances.

3.14 Direct concerns to the Home Manager as the registered Manager where appropriate, including referrals and follow ups to local health teams.

3.15 Ensure that registered manager involved in any move undertake all duties
required by CSCI.

3.16 Review placement in line with the services policies and procedures.

4 Monitoring

4.1 To positively complete all necessary recording, both written and on the
departments IT systems of assessments, care plans and reviews in line with the
workload or as requested by the manager or duty manager.

4.2 Ensure that the manager or duty manager is informed in supervision of all
funding requests or potential care package/placement breakdowns

4.3 To inform the manager or duty manager on any issue on unmet need.

4.4 Undertake and distribute correspondence within the department’s timescales.
Report all complaints to the manager or duty manager ensuring the council’s
timescales are adhered to.

4.5 To work within the services workload monitoring system

4.6 Attend and participate in Vulnerable Adults meetings and conferences where
agreed by manager or duty manager.

4.7 To actively participate in service reviews as requested by the manager or duty
manager.


5. Development

5.1 To be aware of all legislation and changes of legislation relating to disabled adults within the Assessment and Care co-ordination service

5.2 To actively participate in the supervision and appraisal process with the
manager or duty manager in order to assess the progress of work, and to identify development and training needs. To seek advice from other professionals where necessary.

5.3 To participate effectively in team meetings.


5.4 To attend such training courses, workshops etc required, to fulfil the work of
the post and to enhance performance as appropriate to professional development and the needs of the service.

5.5 To participate in NVQ or other qualification based course if available and appropriate.

5.6 To undertake other tasks as necessary which is within the capability and grade of the post holder as directed by the Manager or duty manager.
